Case Management

Recovery does not happen in a vacuum. For many people, opioid use disorder is entangled with housing instability, unemployment, legal involvement, healthcare gaps, and strained family relationships. At Alliance Wellness Clinic, we believe that removing these barriers is as important as the treatment itself. That is why case management is built into your care, not referred out, not optional.

What Case Management Provides

Your care coordinator works alongside your medical provider, counselor, and peer recovery specialist to ensure that the practical dimensions of your life support your recovery. Case managers at Alliance Wellness Clinic help with:

How It Works

When you begin care at Alliance Wellness Clinic, your care coordinator will meet with you early in your treatment to understand your current situation and identify any barriers that might affect your ability to stay in treatment or progress in recovery.

From there, your care coordinator develops a resource plan alongside your clinical team, identifying the most pressing needs, connecting you to the right programs, following up on referrals, and adjusting support as your circumstances change.

Case management is a collaborative, ongoing process. Your coordinator does not disappear after the first referral, they stay involved, track progress, and help you navigate obstacles as they arise.

Confidentiality

All case management services are confidential and protected under HIPAA and 42 CFR Part 2. Any communication with outside agencies or providers requires your written consent, and you remain in control of what is shared and with whom.

Getting Started

Case management is included as part of your care at Alliance Wellness Clinic. Let your counselor, nurse, or admissions staff know you would like to connect with a care coordinator or ask about case management during your first visit.
